I think you'd struggle to do this in a nice way with the current store checkout process. The checkout module is not template driven and no very flexible. if it was one of my clients asking for this then I would probably have an extra module placed in the store which finds out what products are in the cart and displays related products, but you'd need to create this yourself, there is nothing I can think of in the current store to do it!!....maybe Rob has an workaround idea?
I think you do have a good point, this should really be something the store can deal with, I've made a note for v3 of the store, we'll see if it works itself into the release.
