There is nothing wrong with the module running on dnn7, windows 8 or the css files.

It is by design.

The problem is that you have not set up your shipping for the client orders.

The give away is the
"<p>
This order requires special delivery.<br />
Please contact us at the email address below:
</p>" line

If you set up your shipping for the orders you will then see the "order button".

Make sure you, in shipping methods, check "non-dealer" so the shipping method will be used for none dealers, of course if you have dealers make sure you check the "Dealer" checkbox.
