I think you'd have to use two options to achieve something workable...
Create one that says something like "Add your own message" and has values No/Yes and where Yes adds the $1.50
And then another option which is the message field.
You might have to modify the template to alter the order of the options so that the message field comes after the first option.
Note that you can also create a default product with ready-made options and then repeatedly copy that when you want to add new products that need the same options. That just helps speeds things up for the operator.
