For the pupil name field you already have on your pages - It looks like you enabled the messagefield option for that.
That message field is provided as sample code in the templates so that you can examine it and do the same again if needed.
Lines 110 to 120 in productdetail.template is where the first product option is switched to a text input when the messagefield.flag is enabled.
You can use the code in line 117 to modify the second product option to do the same thing. You don't have to create the messagefield.flag functionality as that is only there for providing the switch feature.
Take it from there and see how you go.
