I think your idea above should work, but might take a little working out to get the best results. I think maybe you don't even need to change the source code. Just make a empty dropdown list in your product templates, with some kind of identifying class so jQuery can pickup the control and then call your webservice.If I use an empty dropdown - How will the server pickup the value in put it in the cart description?. I think currently the Cart is picking up controls that starts with "option" that were initially created by the server.

You might also want some way of hiding the control for products that don't us it.
But like I said before I don't; think you'll be able to link this into the stock control and also adding to the basket might cause you some issues.
