I think your idea above should work, but might take a little working out to get the best results. I think maybe you don't even need to change the source code. Just make a empty dropdown list in your product templates, with some kind of identifying class so jQuery can pickup the control and then call your webservice.
If I use an empty dropdown - How will the server pickup the value in put it in the cart description?. I think currently the Cart is picking up controls that starts with "option" that were initially created by the server.

Am I missing something?
You might also want some way of hiding the control for products that don't us it.

But like I said before I don't; think you'll be able to link this into the stock control and also adding to the basket might cause you some issues.
