This is a little strange. I've had a client move back to paypal last week and I thought this is my chance to see what's going wrong. But I simply updated the XML config for paypal and everything worked!
I'm running DNN6 so no "advanced" urls...having this turned on, I know can cause issues. The only thing I can suggest is you make sure the web.config is using humanfriendly urls.
It's a number of years since I wrote the paypal code, so it is time for a review. I'll try and find the time to make the paypal code have it's own project in codeplex, and hopefully during the process we'll find out what the issue is.
